Livongo for Diabetes

This free program is available to associates and their family members who are

• enrolled in the Publix BCBS PPO Plan and
• living with diabetes.

• More than a standard meter: The Livongo cellular-connected, touch-screen meter provides real-time, personalized, on-screen tips based on your glucose readings and automatically uploads your readings to a secure and confidential mobile app.
• Coaching and support anytime and anywhere: Certified expert coaches are available through phone, text and the Livongo mobile app to give you guidance and answer questions about living with diabetes. You can also use the app or visit the Livongo website to find other resources for living with diabetes.
• Unlimited glucose test strips at no cost to you: Get as many glucose test strips and lancets as you need at no cost. When you are about to run out, Livongo ships more supplies, right to your door.
